Square notation on 5 red lines with oblique pes. One clef, left-headed custodes (stem downwards).
Decoration
No decoration detected. Rubrics in red.
Inscriptions
Both sides of the folio show several inscriptions, mainly in brown ink: "1643" (twice) and "Nº 60". On the recto side there is also a modern inscription in pencil that reads "N-258".

Damaged. The folio has been cut and folded to fit the size of the book that it covers. The external part of the cover is partially unreadable.
