Square notation on 5 red lines with oblique pes and scandicus. One clef; left-headed custodes (stem upwards).
Decoration
One simple black initials for a verse. Rubrics in red.
Inscriptions
On the recto side there a few inscriptions in brown ink. Two of them both read "1616". On the verso side a modern inscription in pen reads "B-12-3-24"; another modern inscription, in pencil, reads "N-227".

Condition of document
Damaged. The folio has been cut and folded to fit the size of the book covered. The recto side, the external part of the cover, is quite dirty.
