Square notation on 5 red lines with oblique pes and scandicus. One clef, left-headed custodes (stem downwards).
Decoration
Part of a blue initial with red filigree is detected. Rubrics in red.
Inscriptions
On the verso side a modern inscription in pencil reads "B-12-3-28", another one in pen reads "N-231". On the recto side there is an inscription in brown; it can be read: "23 de janeiro de 1623 / 14 de Outubro de 1624 / Offº de Matheus de Freitas / Pero Novaes Soares".

Damaged. The folio has been cut and folded to fit the size of the book that it covers. The external side of the cover (recto) is quite dirty.
