Square notation on a five-line red staff. The custodes are barely discernible.
One red initial with blue penwork flourishing. A simpler black initial marks the verse. Rubrics in red.
Several inscriptions are present on the recto: "Bapt 1630 te 1657".
A paper label is pasted onto the parchment: "Sande (S. Martinho). Nasc. 25/8/1630 - 14/11/1654 Cas. 13/1/1633 - 25/11/1654 Obt 5/12/1630 - 21/9/1675".
On the same side, but visible from the verso owing to the fold of the parchment, two modern inscriptions in pen and pencil read "B-16-2-35" and "P-702".
Damaged. The folio was reused as a book cover and was therefore cut and folded to fit the dimensions of the volume. The outer surface (recto) is heavily soiled and only barely legible.
Single column, preserving at least seven lines of music alternating with seven lines of text.
No foliation/pagination.
