Two stickers on the recto and the verso: "1551".
Recto: Pencil inscription (top left): "28". Right top corner: Postmark of the library. Stamp: "Biblioteca / do / Liceu Dr José Falcão / N.° geral / Est. 16 / Tab.° 7 / N.° 11". [Liceu Dr José Falcão = Escola Secundária D. João III]. Non readable scribbles between columns.
Verso: top margin, in pencil: "Kyriale - circa sec. IX-X"

Description of the notation: 

Square notation with English features (substitution of lozenge for square punctum) on four red lines. F and C clefs. A few custodes were added by a second hand.
